Job Objective: The Supportive Housing Case Manager provides strengths-based case management utilizing the Pathway of Hope Case Management Model to maintain housing, provide resources, and preventing a return to homelessness. What You Will Do:
Minimum Qualifications: The requirements listed below are representative of the minimal education, experience, skills, and/or abilities required for this position. Education: Bachelor's degree in social work, psychology, sociology, or related field.
Background Checks: The position requires a background check to be completed. Findings may disqualify an individual for this position
Experience: Two years providing case management and support services in a social service setting. Assisting vulnerable populations is preferred. Knowledge of Housing and Urban Development standards preferred. Certifications: Must have a valid driver's license and maintain The Salvation Army Driver's qualification standard; must complete Safe from Harm training within the first 90 days of employment; completion of Case Management Certification Program within 120 days of employment; completion of SIMS data training within 60 days of employment. Travel: May require travel for training and local travel for meetings. Working Conditions: Work is performed in an office environment located in a residential building; may require some weekend and evening work.
